This issue was reported to Lotus Quality Engineering and has been addressed in
Notes 5.0.4.

SPR# CDCO4G4S5A - Fixed a crash that occurred running an action that ran
@toolsrunmacro, where the action was from a subform within a subform. [5.0.4]
Workaround: Use a Shared Action instead of programming the Action directly
